ICMR ART 2021 Documentation Requirements

  • Couple consent for IVF/ICSI — signed by both partners with identity verification.
  • Informed consent for ovarian stimulation and OPU — risks, alternatives, and procedure explained.
  • Embryo disposition consent — what happens to unused embryos (cryopreservation, discard, donation for research).
  • Cryopreservation consent — duration, storage fees, and conditions for thaw.
  • Donor consent (for donor cycles) — separate consent for oocyte or sperm donors.
  • Financial counselling acknowledgement — patient has received a written breakdown of cycle costs and package terms.

Under ICMR ART 2021, these consents must be in English or the patient's language, dated and signed before cycle initiation, retained for a minimum of 10 years, and available for inspection by the ART Authority.
